This is a Cornelius "ultra detail" figure from the 2001 film
Planet of the Apes
.  The figure is about 5.75" tall and is still brand new and factory sealed.  The packaging has some nicks, scuffs, scratches, light bends, and light edge wear, though is still overall in pretty good condition.
